It's some sort of split/separation that they used a couple times.

I doubt there's just one thing going on there. It looks like some sort of posterization that's been offset, time-stretched and blended with the original. She might also have been luma-keyed, turned monochrome and comped back over itself. Or someone just hit the "Blech" button...
